OKLAHOMA A.S.A. STATE TOURNAMENT
ENTRY FORM
DRESS CODE:
All teams must dress according to the dress code of the Oklahoma A.S.A. in like shirts (body not altered), same color and style with a minimum 6” Arabic number in a contrasting color. (Exception—Men are not required to wear hats but if they do the hats must be worn properly.) Teams advancing from State must dress according to the Regional or National dress code.
SOFTBALLS:
SOFTBALLS WILL BE PROVIDED BY WORTH BALL MFG.
TWO softballs will be given to each team at check-in. Teams may provide additional WORTH softballs at their own expense.
ENTRY DEADLINE:
1. Men’s and Women’s Slow Pitch Church State tournament entry deadline
is July 18, 2008.
The entry fee is $225 for non-sanctioned teams. $200.00 for sanctioned teams.
2. Women’s Slow Pitch State Tournament entry deadline is July 18, 2008

3. Co-ed Church Slow Pitch State Tournament deadline is August 1, 2008.
